1. VA officials in Phoenix created a secret wait list for veterans.
2. The list made it look like there wasn’t a long wait for care.
3. Some veterans waited months or years to get care.
4. 40 veterans may have died as a result of the list.
5. A national investigation was launched.
6. The Inspector General’s report was released on 8/26.
7. Report says no veterans died because of the list in Phoenix.
